1. Eligibility:This promotion is open only to licensed dental hygienists who are legal U.S. residents residing in one of the 48 contiguous United States or the District of Columbia and are 18 years of age or older at time of entry (except residents of AL & NE who must be 19 years or older at time of entry and residents of MS who must be 21 years of age or older at time of entry). To be eligible, dental hygienists must be in a clinical practice setting located in the 48 contiguous United States or the District of Columbia. Employees of Procter & Gamble, its affiliates, subsidiaries, advertising, promotion and internet agencies and their immediate family members and/or those living in the same household of each are not eligible. All federal, state, and local laws and regulations apply. Void where prohibited by law.
2. Sponsor:The Procter & Gamble Distributing Company LLC, One P&G Plaza, Cincinnati, OH 45202.
3. Agreement to Official Rules:Participation in this Contest constitutes entrants and their nominees, potentially selected as winners, fully and unconditionally agree to these Official Rules and Sponsor’s and Administrator’s decisions, which are final and binding in all matters related to this Contest. Becoming a winner is contingent upon fulfilling all requirements set forth herein.
4. Timing:The Contest begins on July 28, 2011 at 12:00 a.m. Eastern Time ("ET"), ends June 4, 2012 at 11:59 p.m. ET (the "Contest Period"). Sponsor’s computer is the official time-keeping device for the Contest.
5. How to Enter
   1. Limit one entry per person per nominee throughout the duration of the promotion, regardless of method of entry. If more than one entry for said nominee is received from said person throughout the duration of the promotion, only the first such entry received will be eligible. However, one person can nominate more than one nominee.
   2. All entries must be submitted via written form as online submission or mail-in entry. Sponsor not responsible for lost, late, illegible, incomplete, misdirected, postage due, or mutilated mail or entries, or for errors, delays or failures in transmission of online entries.
   3. All nominees must exhibit the following :
      1. RDHs with 2+ years of practice experience after graduation from dental hygiene school
      2. RDHs with community service involvement
         • General volunteer (non-oral health-specific) examples welcome
         • Oral health-related volunteer examples preferred
      3. RDHs with examples of work that goes above and beyond the call of duty. Examples include:
         • Excellent patient relations/special care/retainment
         • Involvement in research and/or clinical experience
         • Published work
         • Ability to generate additional business for their practice
   4. Entries will not be returned after submission
   5. Entering a Submission constitutes entrant’s consent to give Sponsor a royalty-free, irrevocable, perpetual, non-exclusive license to use, reproduce, modify, publish, create derivative works from, and display such submissions in whole or in part, on a worldwide basis, and to incorporate it into other works, in any form, media or technology now known or later developed, including for promotional or marketing purposes.
6. Judging:A total of six winners will be selected throughout the year. The first five winners will be selected by an independent committee of judges whose decisions are final. The sixth round of finalists will be featured on the Crest Oral-B Professional Facebook page. Fans of the Crest Oral-B Professional Facebook page will vote to determine the final winner. Voting for the winner will be closed and the winner determined no later than the week of June 4, 2012. Entries will be judged on the following criteria and a telephone interview by one or more of Procter & Gamble’s Crest/Oral-B representatives:
   1. RDHs with 2+ years of practice experience after graduation from dental hygiene school
   2. RDHs with community service involvement
      1. General volunteer (non-oral health-specific) examples welcome
      2. Oral health-related volunteer examples preferred
   3. RDHs with examples of work that goes above and beyond the call of duty. Examples include:
      1. Excellent patient relations/special care/retainment
      2. Involvement in research and/or clinical experience
      3. Published work
      4. Ability to generate additional business for their practice
   All six winners will be notified via e-mail throughout the year with the final winner notified no later than June 1, 2012. Each winner must sign and return any required Affidavit of Eligibility/Release of Liability and Publicity form within five (5) days of delivery to such contestant’s address as identified on the entry submitted by his/her nominator. Non-compliance within this time period will result in disqualification and an alternate contestant will be selected. Any contestant notification returned to Sponsor as undeliverable will result in disqualification and the naming of an alternate contestant. Decisions of the judges are final in all matters relating to this contest. No substitution or transfer of prizes permitted. All federal, state, and local taxes on prizes are the sole responsibility of the winners. All federal, state, and local laws and regulations apply. Acceptance of prizes offered constitutes permission to use each winner's name and/or likeness in advertising and promotional activities without additional compensation.
7. Notice to Online Entrants:ANY ATTEMPT BY AN INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION IS A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW. Not responsible for faulty, incorrect or mistranscribed phone transmissions, incorrect announcements of any kind, technical hardware or software failures of any kind including any injury or damage to any person's computer related to or resulting from participating in or experiencing any materials in connection with the promotion, lost or unavailable network connections, or failed, incomplete, garbled or delayed computer transmission that may limit a user's ability to participate in the promotion. Sponsor reserves the right to cancel or modify the promotion if fraud, misconduct or technical failures destroy the integrity of the program; or if a computer virus, bug, or other technical problem corrupts the administration or security of the program as determined by Sponsor/judging agency/administrator, in their sole discretion. In the event of termination, a notice will be posted online and the drawing for all remaining prizes will be conducted from among all eligible entries received prior to termination. Any damage made to the website will be the responsibility of the authorized e-mail account holder of the e-mail address submitted at the time of entry. Proof of submitting entries will not be deemed to be proof of receipt by Sponsor. Any entries which are suspected of being fraudulent (including those using robotic, automatic, programmed or similar methods of participation) will be disqualified, based on determinations made solely by Sponsor. Sponsor reserves the right to prohibit the participation of an individual if fraud or tampering is suspected or if the individual fails to comply with any requirement of participation as stated herein or with any provision in these Official Rules.

   2. Recognition at a special award cocktail reception at RDH Under One Roof 2012 in Las Vegas, NV
      1. Includes RDH Under One Roof conference registration ($295 for member) Travel & Accommodations: Sponsor will coordinate all travel and on-site accommodations (including stay at the RIO All-Suite Hotel and Casino) up to $2,500. If travel by air is required, an economy class flight will be arranged. Ground transportation to and from the airport in the winner’s city is also included.
   3. Recognition in dental trade media news announcements and announcements on dentalcare.com
   4. $1,000 monetary prize
   5. Exclusive trip to P&G headquarters
9. General Conditions:Sponsor reserves the right to cancel, suspend and/or modify the Contest, or any part of it, if any fraud, technical failures or any other factor impairs the integrity or proper functioning of the Contest, as determined by Sponsor in its sole discretion. If terminated Sponsor may, in its sole discretion, determine the winners from among all non-suspect, eligible Submissions received up to time of such action using the judging procedure outlined above. Sponsor, in its sole discretion, reserves the right to disqualify any individual it finds to be tampering with the Submission process or the operation of the Contest or to be acting in violation of these Official Rules or those of any other promotion or in an unsportsmanlike or disruptive manner and void all associated Submissions. Any attempt by any person to deliberately undermine the legitimate operation of the Contest may be a violation of criminal and civil law, and, should such an attempt be made, Sponsor reserves the right to seek damages and other remedies (including attorneys’ fees) from any such person to the fullest extent permitted by law. Sponsor’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.
10. Video:A professional video crew will travel to the winners’ hometown to shoot a video during an agreed upon timeframe. Each winner will identify colleagues, patients, etc. to participate in the video.
11. Release: By participating, entrant/nominee agrees to release and hold harmless the Sponsor and their respective subsidiaries, affiliates, suppliers, distributors, advertising/promotion agencies, and prize suppliers, and each of their respective parent companies and each such company’s officers, directors, employees and agents (collectively, the “Released Parties”) from and against any claim or cause of action, including, but not limited to, personal injury, death, or damage to or loss of property, arising out of participation in the Contest or receipt or use or misuse of any prize.
12. Limitations of Liability:Released Parties are not responsible for: (1) any incorrect or inaccurate information, whether caused by entrant, printing, typographical or other errors or by any of the equipment or programming associated with or utilized in the Contest; (2) technical failures of any kind, including, but not limited to malfunctions, interruptions, or disconnections in phone lines or network hardware or software; (3) unauthorized human intervention in any part of the Submission process or the Contest; (4) printing, typographical, technical, computer, network or human error which may occur in the administration of the Contest, the uploading, the processing or judging of Submissions or votes or the tabulating of votes, the announcement of the prizes or in any Contest-related materials; (5) late, lost, undeliverable, damaged or stolen mail; or (6) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the Contest or receipt or use or misuse of any prize (including any travel/activity related thereto). Released Parties are not responsible for misdirected or undeliverable Submissions or for any technical problems, malfunctions of computer systems, servers, providers, hardware/software, lost or unavailable network connections or failed, incomplete, garbled or delayed computer transmission or any combination thereof. Released Parties are not responsible for any unauthorized third party use of any Submission.

14. Winners List:Winners List will be published throughout the contest. A total of six winners will be announced throughout the year with the final winner announced no later than the week of June 4, 2012.
