1. ELIGIBILITY: This promotion is open only to residents of the 50 United States and the District of Columbia who, at time of entry, are part-time or full-time dental hygienists or dentists, 21 years or age or older and have received a Crest Pro-Health Paste 14-Day Challenge Invitation from their Crest Territory Manager. Employees of Procter & Gamble, their affiliates, subsidiaries, advertising, promotion and internet agencies and their immediate family members and/or those living in the same household of each are not eligible. All federal, state, and local laws and regulations apply. Void where prohibited by law.


2. HERE’S HOW TO REGISTER AND PARTICIPATE IN THE CHALLENGE: Crest representatives will be inviting selected dental professionals to participate in the Crest Pro-Health Paste 14-Day Challenge. If you receive an invitation and samples of the Crest Pro-Health Paste (and meet all eligibility requirements listed in Rule #1), beginning 12:01 am (ET) on 2/25/08 through 11:59pm (ET) on 6/15/08, visit www.dentalcare.com/prohealthchallenge and follow the on-screen registration instructions including providing the Official Invitation Code indicated on your Crest Pro-Health Paste 14-Day Challenge Invitation. You will earn one credit towards a sweepstakes entry on the day of your registration (Day 1). Then, use the Crest Pro-Health Paste on Day 1 and for the next consecutive 13 days. Each day that you log-in and provide your answers to the survey questions, starting with Day 2 through Day 13, you will earn one credit towards an additional sweepstakes entry. You must, however, submit your answers to the final completion survey at least 13 days after registering and by 11:59:59 pm (ET) on 6/30/08, in order for any entry(ies) to be included in the sweepstakes (maximum of 14 total entries per person). You will not earn entry credits for any days you do not log-in and provide your answers to the survey questions. Limit one entry per person per day. If more than one entry is received per person per day, only the first such entry received will be eligible.


3. RANDOM DRAWING: A random drawing will be conducted on or about 7/7/08 from among all eligible entries received by 6/30/08. Random drawing will be conducted by D.L. Blair, Inc., an independent judging organization whose decisions are final on all matters relating to this offer. Odds of winning will depend upon the number of eligible entries received.


4. PRIZES: Ten (10) prizes, with a total approximate retail value of $35,000 will be awarded as follows: (10) Grand Prizes – A 4-day/3-night trip for 2 to the 2008 American Dental Association Convention in San Antonio, TX (10/16 – 10/19/08), including round-trip coach air transportation from major airport nearest winner’s home, hotel accommodations (double occupancy), ground transfers in San Antonio, registration and 4-days’ admission to the ADA convention for winner, and $600 spending money for winner (Approximate Retail Value: $3,500 ea.). Winner and guest are solely responsible for all other expenses not specifically set forth herein, including but not limited to meals, additional transportation, souvenirs and other incidentals and items of a personal nature. The actual retail value of the prize will depend on winner's point of departure/return. Winner and guest must travel on same itinerary. In the event a Grand Prize winner’s residence is within a 150-mile radius of San Antonio, TX, he/she will receive a $200 stipend in lieu of air transportation and ground transfers. Prizes consist of only the items specifically listed as part of the prize. Limit one prize per person or household.


5. GENERAL RULES: Potential winners will be required to complete and return an Affidavit of Eligibility, Release of Liability, Prize Acceptance Form and, if legally permissible, a Publicity Release within 10 days of attempted delivery of same. Noncompliance within this time period or return of any prize/prize notification as undeliverable will result in disqualification and an alternate winner will be selected, if time permits. Winners will be notified by mail. Traveling companion must also complete and return a Release of Liability within 10 days of winner’s attempted notification. If a minor is selected as a traveling companion, winner must be his/her parent/legal guardian. All travelers must possess required travel documents (e.g., valid photo ID) prior to departure. All travelers must agree to depart and return on dates specified by the Sponsor; dates of departure and return are subject to change. No substitution or transfer of prize by winner permitted. Sponsor reserves the right to substitute prize of equal or greater value. All applicable taxes are the sole responsibility of winner. In no event will more than the stated number of prizes be awarded. If a potential winner’s employer’s policy prohibits an employee from receiving a prize, an alternate winner will be selected. In the event of an internal company dispute regarding specific employee entitled to receive prize, the decisions of the employer’s owner/operator are final. By accepting prize, winners agree to hold Sponsor, its respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of use of prize. Winners assume all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize. Acceptance of prize constitutes permission for the Sponsor and its agencies to use winners’ names and/or likenesses for purposes of advertising and trade without further compensation, including a winners’ list, unless prohibited by law. Entrants agree to release, discharge and hold harmless Sponsor, its respective directors, officers, employees and assigns, harmless from and against any and all liability and damages. By participating in this promotion, entrants agree to be bound by the Official Rules and the judges’ decisions which are final. In the event there is a discrepancy or inconsistency between disclosures or other statements contained in any sweepstakes materials and the terms and conditions of the Official Rules, the Official Rules shall prevail, govern and control. All material submitted becomes the sole property of Sponsor and will not be returned. Sponsor not responsible for any typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of the prizes.
