An avulsed permanent tooth should be replaced quickly within seconds or minutes. The longer the tooth is out of the mouth, the poorer the prognosis. Do not handle the root surface, but do rinse the tooth quickly under running water to remove foreign material.

If it is not possible to reimplant the tooth immediately, it should be stored in milk or saline for transportation to the dentist. Treatment may include application of a splint following reimplantation to stabilize the tooth, prescriptions for systemic antibiotics, and oral antimicrobials and pulp therapy.

Most displaced permanent teeth undergo pulpal necrosis and will require the initiation of root canal therapy one week after stabilization.
