Patient Profile: The patient is a 10-year old African-American male, 4' 5" and weighs 90 lbs.

Chief Complaint: "I want my teeth to look better and not hurt."

Medical History: Allergic to Penicillin. No other known allergies. Patient had chickenpox at 4 years of age. He has received childhood vaccinations.

Dental History: The patient has visited the dentist only when in pain.

Supplemental Information:

  • He uses a manual toothbrush and does not brush daily or use fluoridated toothpaste.
  • He is apprehensive about dental treatment.

Social History: Family is from a low socio-economic background. The patient is in the 4th grade and loves to play baseball and basketball.
