Patient Profile: Patient is a 50-year old White Caucasian male in good health.

Chief Complaint: "I am very concerned about the stain on my front teeth."

Dental History: The patient last saw the dentist for a crown three years ago and received a prophylaxis eight months ago.

Medical History: The patient suffered a slipped disc in his neck 10 years ago and complains of shoulder pain while being reclined for long periods of time. He wears contact lenses.

Extraoral Examination:

  • The patient's facial symmetry, lips, skin and larynx were within normal limits.
  • TMJ crepitus was present on the right side only and no problems were indicated by the patient.

Supplemental Information:

  • Patient reports to have dry mouth frequently.
  • Patient is curious how to whiten his teeth.
  • A full series of radiographs were exposed four years prior to the September probe readings.

Health Behaviors:

  • Patient brushes twice daily with a fluoridated dentifrice.
  • Uses a phenol mouthrinse daily.
  • Does not floss regularly.

Social History: He is very energetic, smokes one pack of cigarettes daily and has 2-3 alcoholic drinks weekly.
