Safe techniques include the maintenance of firm control of the instrument at all times, exchanging instruments only in the transfer zone over the patient’s chest and avoiding unpredictable movements during the transfer. Laying instruments or material on the patient’s napkin should be avoided to prevent injury and potential litigation.
