Rehabilitation Programs
Treating methamphetamine use disorder presents unique challenges. Patients frequently experience poor treatment engagement, high relapse rates, prolonged cravings, depression, anhedonia, cognitive impairment, anxiety, and stimulant-induced psychosis. Deficits in attention, memory, executive function, and impulse control may reduce the effectiveness of traditional educational approaches and make long-term recovery particularly challenging.1,20,40
Treatment should be individualized to address each patient's medical, psychological, and social needs. Certain populations—including women, men who have sex with men (MSM), and individuals with co-occurring psychiatric disorders or histories of trauma—may benefit from specialized treatment approaches tailored to their unique clinical circumstances.20,43
Successful treatment typically involves more than detoxification alone. Withdrawal management may represent the first stage of care, but long-term recovery generally requires comprehensive behavioral treatment combined with ongoing recovery support.1,27
Levels of Care
Treatment may occur in a variety of settings depending on the severity of illness and the patient's individual needs.
Inpatient or residential treatment
provides 24-hour medical supervision, behavioral therapy, counseling, and recovery support.
Outpatient treatment
allows patients to participate in structured therapy while continuing to live at home or in a recovery residence.
Continuing recovery support
may include peer-support programs, family involvement, community resources, and long-term recovery services.1,27
Evidence-Based Behavioral Treatment
Current evidence supports behavioral therapy as the foundation of treatment for methamphetamine use disorder. The strongest evidence currently supports:

These interventions are frequently combined and supplemented with relapse-prevention strategies, family therapy, peer-support programs, and long-term recovery planning.
One of the best-studied behavioral interventions is the Matrix Model, a structured 16-week treatment program that combines cognitive behavioral therapy, family education, individual counseling, drug testing, 12-step participation, and encouragement of healthy non-drug-related activities.18,40
Mutual-help organizations, including Crystal Meth Anonymous (CMA) and other 12-step programs, may provide valuable long-term peer support for many individuals in recovery. Although evidence continues to evolve, these programs remain useful adjuncts to formal behavioral treatment for many patients.18,42
Contingency management is currently considered one of the most effective behavioral interventions for stimulant use disorder. This approach uses positive reinforcement to encourage abstinence, treatment participation, and completion of recovery-related activities. Voucher- and prize-based reinforcement systems—including the well-known "fishbowl" method—have demonstrated significant improvements in treatment retention and abstinence.1,9
Research also suggests that shorter, more frequent counseling sessions may improve learning and retention because chronic methamphetamine use can impair attention and memory.21
Emerging Treatment Approaches
Technology-assisted care—including telehealth, web-based treatment platforms, and mobile applications—has expanded access to behavioral treatment, particularly for individuals living in rural or underserved communities. These approaches may improve treatment engagement, continuity of care, and long-term recovery support.1,41
At present, no medications have been approved by the U.S. Food and Drug Administration (FDA) specifically for the treatment of methamphetamine use disorder. Although combinations such as extended-release naltrexone with bupropion and several other medications have demonstrated encouraging results in selected clinical trials, behavioral interventions remain the cornerstone of treatment. Ongoing research continues to evaluate additional pharmacologic therapies, including immunotherapies and medications targeting neuroinflammatory pathways.12,40
Emergency management continues to focus on stabilization of acute intoxication, including treatment of agitation, hyperthermia, cardiovascular complications, seizures, airway compromise, and other life-threatening conditions. Following medical stabilization, patients should be referred for comprehensive treatment and ongoing recovery services.1,20
