Post-Operative Management
Patients with bleeding disorders must be observed longer than the average patient to monitor for post- operative complications.
For post-operative management of patients with bleeding disorders, the dental provider should consider the following:
To achieve good hemostasis use sutures, topical hemostatic agents, and mechanical pressure when indicated.
In the case of dental extractions or surgery, instruct the patient to apply physical pressure on gauze pre-treated with hemostatic agents to observe for proper hemostasis.
Keep the patient in the office longer to observe for proper hemostasis.
In the case the patient develops a liver clot, a result of the continued disruption of the clotting process, the patient will need additional factor infusion. The dental provider will need to consult with the patient’s hematologist/HTC to determine a treatment plan for debriding the site to remove any material/foreign bodies contributing to the formation of the liver clot inclusive of additional factor infusion.
Schedule surgical procedures early in the week in case a post operative or emergency issue should arise.
